Report a hazard, and deal with it
Anyone reports from their phone; the safety contact records what was done.
What this is for
Anyone who spots something that could hurt someone reports it in a few taps on their phone, with a photo. It goes straight to the shop's safety contact, who records what was done and closes it, and the person who reported it can see it's been dealt with.

Hazards list

- What's waiting, how many are high or extreme, how many are made safe but not fixed for good, and what's been closed. Staff see their own count instead of Closed.

- Filters. Open hazards are listed worst first.
The report form
- What's the hazard? · Where is it?
- A line is enough.
- How bad does it look?
- Low, Medium, High or Extreme. Your best guess; the safety contact can change it. Extreme reminds you to stop work near it and tell your supervisor in person as well.
- Photos · Send the report
- Opens the phone camera. The report sends first and the photos follow, so a weak signal never loses it.
One hazard
- What was reported · Photos
- The report, where, how bad, who reported it and what's been done.
- Deal with it (safety contacts)
- What's been done (the reporter sees this), How bad it is, and How it was dealt with: Fixed, Controlled, Added to the risk register, or Checked, nothing needed.

- Made safe for now needs what's been done; Close it needs that and how it was dealt with. The reporter is told either way.

Before you start
- Your shop is on Workshop or Scale. Staff can report if your shop lets them; it does to start.
Step by step
- 1
On HR & Safety, press Report a hazard.
- 2
Say what it is and where, choose how bad it looks, and take a photo.

Press Send the report. It goes to the safety contact straight away.
- 4
Safety contact: open it from the notification or Hazards, write what's been done, and choose Made safe for now if it still needs a proper fix.
- 5
Once it's sorted, choose how it was dealt with and press Close it. If it belongs in the risk register, add it there.
Tips & gotchas
- If someone could be hurt right now, stop work near it and tell someone in person too. The report isn't a substitute.
- Everyone can see hazards that are still open. Closed ones stay with the safety contacts, HR and whoever reported them.
